I think they are using version 2.x hl7, which still lets them extract fields such as request id or even patient id from results so they can be matched to local request id when received. It's easier if you have the opportunity to do your own message format protocol, such as bittorrent did with python , because everyone was sharing the same end points ( bittorrent peers).
