Now that I reconsider this question, I am realizing that "item" is here a very 
nonspecific term... I suppose the most general meaning would be

   billable "something(s)", most often in the context of an encounter

where the encounter type may serve as a proxy but *probably* containing 
(clinically) services and/or consumables for which billing needs to be done.
