I think, we have used \small only in soap table. I would try and redefine small 
using package type1cm. It allows redefining the font sizes.  It would be 
interesting to see the effect.

Well, when it's re-defined inside the template it doesn't
matter what *I* think -- all that matters is what YOU want -
patient-centered Informed Shared Decision Making :-))

One Caveat: depending on where exactly inside the template
\small is redefined that will affect the entire scope :-)

IOW since GNUmed itself is creating the LaTeX code for the
SOAP table of that placeholder you can only re-define \small
outside that placeholder which means, yes, it will make
\small different inside the placeholder but, no, you cannot
affect only one of the \small's inside the
placeholder-generated LaTeX code...
