OK. It's just happened again, so this time I did a bit more
investigation. Checking the frame parameters for the invisible
frame I get :-

((display . "") (visibility . t) (icon-name) (window-id . "8586146")
 (top + -31960) (left + -31998) (buffer-list #<buffer .emacs>
 #<buffer *Minibuf-1*> #<buffer main.cpp<4>> #<buffer MetaServer.idl>
 #<buffer MetaServerI.cpp> #<buffer MetaServerI.h> #<buffer Q_MetaServerI.h>
 #<buffer molly.make> #<buffer main.cpp<2>> #<buffer cmailcheck.cpp>
 #<buffer cdataif.cpp> ...) (unsplittable)
(minibuffer . #<window 4 on *Minibuf-0*>) (modeline . t) (width . 20) (height
. 75) ...)

The top & left parameters look very odd. Doing a
 (set-frame-position <frame> 100 100)
on this frame made is visible, but at the minimum size
i.e. a title bar just big enough to display the title and icons.
