There is a typo in the manual. Correct values of LPX_K_SCALE are:

0 - no scaling;
1 - equilibration scaling;
2 - geometric mean scaling;
3 - geometric mean scaling, then equilibration scaling.

Until 4.2 the default value was 3. In 4.2 the default value is 1.
It was changed because equilibration scaling seems to be much more
appropriate for badly scaled problems.
