Hello Tullio,

Thanks for the report.

    Index contains "Chapter 1: Index" at the top of the page.

This doesn't happen for me with the "short sample".

What version of texinfo.tex are you using?
Please try getting the latest from ftp://tug.org/tex/texinfo.tex and see
if it helps.  If not, please send me the .log file written by TeX as
well as the .texinfo.
