I'm forwarding some questions that have been sent to me by Stéphane
Rivière to see if others have ideas.

Is it possible to split a cartouche across pages?

My suggestion was to @input an automatically generated file with the
line numbers added. I thought it would be possible to add the line
numbers with TeX, but that wouldn't help with the other output
formats. If TeX could run and use the output from an external program
that would be another option, although I doubt that is a feature.
