"Connection refused" means that xinetd isn't listening for connections
to the cvspserver port -- it doesn't have anything to do with your
password.  Check your syslog for error messages from xinetd and/or run
it in debugging mode.

The server gets it's root setting from the client; specifying -d has no
effect.

I'm not sure what you mean.  You can use $CVSROOT/CVSROOT/passwd instead
of, or in addition to, the system password file, and there have been a
number of perl scripts posted here to help maintain that file.
