You don't need to stop the server, though it is an option.

Backups can be made with the server running but you might back up lock
files and partial commits.

I usually run cvslock to readlock the repository, rdist it to another
location, unlock _both_ copies, then backup the copy of the repository
to tape.  This locks the repository to write operations for the duration
of the rdist, but rdist can be quite fast, even with large
repositories.  I also do this in the middle of the night to help avoid
blocking write operations even temporarily.
