Thanks!  I hadn't even considered that a modern installation would not have SSH setup by default.

Port 514 is used for rsh (as opposed to port 22 for ssh).  Recent
releases of CVS use ssh by default for :ext:, so you're either using an
old release of CVS, something other than the standard CVS client, or you
have $CVS_RSH set to rsh.  Forwarding port 514 should have worked if the
CVS server allows rsh access, but that is frequently disabled for
security reasons, which is probably why it didn't.
