I know this is an old thread, but if you are enquiring about CVSNT not CVS, 
then please contact the vendor (yes, I work for the vendor).

But the answers already given are basically correct - you run CVS.EXE as a 
sub-process - but CVSNT does have C++ API (CVSAPI) and we have a .NET API that 
we use internally (eg: for our VSIX provider) to make running it in a 
sub-process a bit easier:
