As a simple user, I have to say I fully agree with you.
(n#)O is simply unacceptable, and
#O can be confusing (it is sometimes used though -- but each time I
see it in a score I'm just puzzled). Besides, what good would be the
'cautionary style if no cautionary accidental is printed in such a
case?
