Hi Valentin,

Attached is a revised patch which works as before, except that I've
had to revert most of the changes I made in pitched-trill-engraver.cc
since I'm not yet familiar enough with Rune's changes to implement my
bug fixes using Key_entry.

If you're using pitched trills, you'll find that they're broken again,
but can be forced as a workaround since I've merged that part of the
fixes I made.
