Strange! The common problem is that you don't have a bar line where
the break should appear, but it seems you have already taken care of
that.
Do you have any notes that cross the bar line? I think this might also
prevent LilyPond from breaking the lines.
I hope you use bar checks
http://lilypond.org/doc/v2.0/Documentation/user/out-www/lilypond/Bar-check.html#Bar%20check
to make sure that all the music is in synch.
