Are you sure that your file is called test.ly and not test.ly.txt,
it can be hard to convince notepad not to add the extra extension.
If the file extension really is .ly, then the default action when
you double click on the file should be to run lilypond and open the
result in Acrobat.
You could also try to save the file in the folder
C:\cygwin\home\your_login_name\
Then, click on the "Cygwin" icon to open a cygwin command window
and run the command
lilypond test.ly
which should produce a test.pdf in the same folder.
Note that the programs you listed cannot be called directly
outside cygwin, as you have already noticed.
I hope you have already realized that LilyPond is fairly different
from most other programs you have experienced in Windows and that
you should not expect to be able to do anything useful with it
without reading the full tutorial.
Regarding your other email, n/a mean "not applicable" and
you click on the circle with the arrows (assuming that you are
referring to the package selection in setup.exe).
