Why not take a look at
http://lilypond.org/doc/v2.4/input/regression/out-www/collated-files.html#system-extents.ly
for a starting point. Also, take a look at the definitions of
box-grob-stencil and box-stencil in scm/stencil.scm. It shouldn't be
hard to modify them to get what you request.
