lilypond-book: problems with dvi and pdf output

I'm trying to get a minmal lilypond-book example to run. But the dvi 
output looks weird (missing note heads) and dvips won't work at all (see 
below).

I found some old messages (around 2003 or 2004) regarding to this 
problem, but no solution.

Any idea?
