\time 4/4
\key c \major
c'4 c' c' c' \cadenzaOn cis'4 cis' cis' cis' cis' cis' \cadenzaOff c'4

Hmm, I tried the above, and it seems that there's no measure bar after \cadenzaOff. That's weird . . . There is a natural sign on the last c'4 - ideally, though, shouldn't there be a measure bar there and then a c'4 note without a natural sign? Is this your problem?

Well, I guess the bar-thing is manual (i.e. \bar "|"), but there shouldn't be a
natural sign there, it seems. Notice how the following code, with the handles both of
the accidental shifts differently (i.e. the one after \cadenzaOff, and the first note on
the last measure):

c'4 c' c' c' \cadenzaOn cis'4 cis' cis' cis' cis' cis' \cadenzaOff \bar "|" c'4
c' c' cis' c' c'
