Lilypond-book cutting first staff symbols

I am typesetting a book with musical examples in Lilypond. In the
exercises, for example, I use a \markup to define the number of the
example, but the final text cuts off the number example. The odd thing
is that, in the out folder, with all the junkie stuff, there is two
examples of the same score. One that is ok, one that is cut, as you can
see attached. I don't know why lilypond-book chooses the cut one. I
tried using \betweenLilypondSystem with the blank header, but is not
working.

Does someone knows how to fix this?
