Hi,
LilyPond's afterGrace-function uses a second Voice to get the desired output.
Compile the following unchanged, you may be surprised:
{ \afterGrace a4 b16 }
Though, the second Voice isn't stated explicitely. Which confuses the
Completion_heads_engraver.
Maybe a bug, please report it:

The following works:
\version "2.18.2"
\layout {
\context {
\Voice
\remove "Note_heads_engraver"
\consists "Completion_heads_engraver"
}
}
\new Voice { c'4 \afterGrace a4 \new Voice b16 }
