Try in Terminal
  export PATH=/usr/bin:/bin:/usr/sbin:/sbin

and then
  /opt/lilypond/bin/lilypond

The idea is to cut down as much as possible on the included binaries. The 
MacPorts installer package should have full paths to all includes.
