This is a known bug.  Check archives for my posting with a patch to
PDF backend that fixes this bug (along with another one).  This
particular bug is caused by the fact that PDF spec requires that
startxref keyword and the xref table offset must be on separate lines,
but PDF backend puts them on one line (and cvi fails for it expects an
integer only).

Look for a message from Wed, 22 Apr 1998, re "Re: Win32 Lout 3.12
binaries available".
