(1) Multi-column table of contents.  I've decided to skip this one this
    time around.  Feature fatigue is setting in, and I'm a bit concerned
    about the effect on report tables of contents.

(2) Glossary.  Done.

(3) Index entry prefixes.  Done.

(4) Letters for indexes.  Done.

(5) Letters for glossaries.  Don't understand the need, since glossary
    entries are fewer and more prominently labelled than index entries,
    so I am declining it for now.
