Because monotone has it's own protocol, and because that makes it
possible to run it as a completely standalone server with no
dependency with any other software. More about it further down.

Uhmm, what do you mean? Do you need to open it in a firewall or
something like that? I'd turn to the manual of the hard- or software
that you need to adjust if I was you.
Or are you asking how to start a monotone server? This is what
'monotone serve --help' tells me:
serve ADDRESS[:PORTNUMBER] PATTERN ...
listen on ADDRESS and serve the specified branches to connecting clients
There's a better description in the manual.

Yes. The monotone server can run on any account, including your own.
I use a dedicated account for my serving.

Not with the standard release, but you can use monotone to check out
the branch net.venge.monotone.ssh, which has such functionality
(there's been a lot of commits to it lately, so I've no idea how well
it works).

Yes, in the branch net.venge.monotone.cvssync. I've never used it
myself, so I've no clue how it works.
