Are you absolutely sure that no adc or io pin got more than 3.3 volt, that no servo voltage got more than 6volt and that yapa power input never ever got more than 17v, and that the servo 5v is not connected to the autopilot 5v using the so called switch pads of yapa next to the xbee?

Well i am very disappointed.
I managed to burn a second YAPA2 and i am sure that i did nothing wrong.
It exhibits exactly the same symptoms as the first one but with more symptoms this time
I2C and modem stopped communicating also.
I don't know what is going on anymore.
The 30 to 12v and 5V power supplies are brand new (not the same as in the first incident),
onboard regulators also work fine.
The AP works for some minutes and then it starts to fall apart slowly.
My setup in brief:
LIPO 6S (for now) ->|--> 12V PSU -> YAPA2 Vin
                                |
                                |-> 5v psu -> YAPA2 servo pins so other servos can get 5v
                                |
                                | -> CURRENT SENSOR ---|--> ESC -> MOTOR
     |
    |--> ADC_1 for voltage and ADC_2 for current
