Misc HW patch queue

- Remove sysbus_add_io (Phil)
- Build PPC 4xx PCI host bridges once (Phil)

I did the mistake to rebase on master after testing,
and not test again, and missed a new header must be
added in "hw/i386/kvmvapic.c". I apologize for that
and will respin a v2 once fully tested again.
