I think that’s all fixed now.  I also added copyright and license
notices to “guile.am” and “env.in” in addition to “CODE-OF-CONDUCT”.
The other non-trivial files should all have notices already.

(“bootstrap.sh”, for example, has no such header because it is trivial.)
