New PulseAudio driver for speech-dispatcher

Grr... Sorry about hitting 'send' accidentally.  I've attached the
modified files to this e-mail, and a script called 'copyfiles' that
copies them to the directories where they live.  I've got two patch
files for the Ubuntu package, which I'll post to ubuntu-accessibility.
