Great.

    CASE_INSENSITIVE_FILENAMES.  Is it ok?  Should it be a command line 
    switch or is it ok for to be only a customization variable?

I think it's fine for it to be a conf variable, since it can set
perfectly easily from the cmd line.  Probably preferable.

BTW, reviewing the --help message, I wonder if --set-init-variable
should be renamed to --set-conf-variable, since we always call those
things "configuration variables", not "initialization variables".  We
could support both names if need be.  Wdyt?
