Assertion `key != ((void *)0)' failed.

I got this assertion failure in cvs (1.10 macosx client, 1.11.17 linux 
server) when trying to check in a few files:
Assertion `key != ((void *)0)' failed.

I figured out what was going on. It indicates a bug in CVS's 
case-insensitive file system support.

I was checking head.md3. On the server, in the repository, in the Attic 
directory was head.MD3,v. Once I removed head.MD3,v from the repository 
on the server, the checkin succeeded.

Perhaps someone can use this information to figure out what's wrong 
with cvs and fix it.
