The original thread on this bug is here
<http://lists.nongnu.org/archive/html/bug-cvs/2005-02/msg00047.html>.

Conrad Pino started talking about abstracting the CVS file access
functions in such a way that they would work on Windows and in a POSIX
environment
<http://lists.nongnu.org/archive/html/bug-cvs/2005-02/msg00063.html>.  I
remember corresponding with him about abstracting select too, but he was
having problems telling file descriptors from pipe descriptors or something.

Of course, the easiest thing might be to make use of the abstraction
layer we already have and implement a pipe_buffer_* API under Windows to
take the place of the fd_buffer_* functions when the fd points at a
pipe. Once the new buffer API was written, using it should be easy
enough to insert into start_rsh_server().
