I've found these problems both in FSF Emacs 20 and FSF Emacs 21.1.
XEmacs (at least v21.1) doesn't have these problems.
I use the unclutter program to hide the mouse cursor when it's not
used. This works not well with Emacs, and since it _does_ work with
XEmacs I think it's an Emacs bug.
I'll describe the situation:
When I've got an Emacs window (frame) with input focus from the window
manager, but the mouse cursor is above another Emacs frame, the cursor
in the window I'm typing turns in the hollow box the moment unclutter
kicks in, though input focus remains like it should be. The Emacs
frame with the mouse cursor on it gets the non-hollow cursor as if it
had input focus.
When the mouse cursor is above another application than emacs the
cursor isn't affected.
The value of `focus-follows-mouse' doesn't change anything.
Furtheremore it looks like `cursor-in-non-selected-windows' is always
`nil' for minibuffers: I haven't seen a single hollow cursor there
with Emacs 21.1.
