Better yet, could you try the following image:

http://people.debian.org/~sthibault/tmp/netinst.iso

If that works, it really means your monitor is quite dumb in not
supporting 640x400.  I'm afraid we'll see more and more of those since
OSes don't use these resolutions any more...  I'm thinking about some
changes in the console to use 720x400 instead.
