(ties & Pitch_squash_engraver) Error

I've added some robustness fixes for 2.1.1, but you should move the
Pitch_squash_engraver to VoiceContext.  Tie_engraver looks at the
right note before Pitch_squash_engraver does its job. Then the left
squashed note is different from the right unsquashed note, so no tie
is created.
