For the other questions, the basic answer is, all the changes were made
in response to other bug reports.  It would take a bit of effort to dig
back and find them all.  Does it really matter?  Seems to me all that
matters is where we are now.

Well, it matters in the sense that in some cases, the most obvious fix (from my perspective) is to back out the changes that cause the problems! That is what several of my patches do, anyway.


Anyway, in general, it doesn't make sense to revert code back to the old
version, because the old version had some bug, or I wouldn't have
changed it.  Thus the need for example source to find where the new
version fails.
