I don't think that the author should want something about the formatted
output whenever the language has some logical markup (as HTML does).
He could want some special HTML but not a given visual result. This
goes against the philosophy of HTML, in my opinion. If he (we) want 
something visual he (we) should use CSS properly.

I still think that it should be the rendering engine (here the browser)
which choose what to do with those markups. For TeX it is different, 
as Texinfo does the rendering.
