I'd say always warn.  (If not error.  Such a construct is uselessly
confusing.  We don't want downstream readers to have to handle such
craziness.)

    2) Should texi2any warn about an empty label in menu, such as
    ...
    (yes, always)

Agreed, always warn.

    3) Should texi2any warn about a case with no space after and or 
    before : in menu
    * a:vvv.
    (no)

I agree, I guess there is no need.  The Menu Parts node doesn't
explicitly say there has to be a space after the colon, so maybe no
change is required, although I suppose we could say something like "a
space is conventional but not technically required".
