These elements contain index: appendix, article, book, chapter, part,
preface, sect1, sect2, sect3, sect4, sect5, section.

<chapter> is in the list so what is the problem?

Is that page I linked to out-of-date? The date there is 2002; if it's
no longer valid is there another reference I can refer to?
