I found that there would always be a warning "pdfTeX warning (dest): name{} has
been referenced but does not exist, replaced by a fixed one" with
pdftex. I tracked it down to these changes.
The use of \countB appears to conflict with the use of \countB in
\pdfgettoks. I didn't fully understand how it make it break, but
changing \mtadjustprotcode to use its own count register made this
warning go away.
