would fetchmail/procmail/gnus+nnmail or something be an option for
you?  If so look at http://sources.redhat.com/cygwin.  Cygwin contains
fetchmail and procmail (if you want as well).  In the
address@hidden archives from the last week or so you should find a
long thread about fetchmail in daemon (i.e., ehm, 'multiuser') mode on
cygwin.

If emacs' smtp features are not sufficient for you, cygwin contains
'ssmtp' and there were reports of succesful exim installations, if I
remember correctly.
