Well, the "FreeBSD version of CVS" :) has such an extension.  See
the "Current local changes:" section in /usr/src/contrib/cvs file
FREEBSD-upgrade.  Maybe some of these changes can make it back
into the standard CVS distro.  There's another current thread
dealing with this topic ("diff in commit message").
