If I bypass the attempt to obtain textwidth and exampleindent in book_textinfo.py in lines c. 207-223, and use the defaults instead, then lilypond-book runs fine. This was with 2.15.21, which I happened to have installed at the time. I think this might have been the fix Phil offered earlier.

At least it is now clear what needs to be reworked to fix this problem. There may be a clue in lilypond-book.py, line 394 et sec, following the comment there, "Because of win32 compatibility, we can't simply use subprocess", although that bit of code seems to be doing something different.
