No. -changes_pending "yes" just compensates that you did not
make any change of the image content. Without the explicit
confirmation of -changes_pending, xorriso would refuse to
start ISO production.

Shall i begin to riddle where this comes from ... ?

Maybe the MD5 checksum array which gets stored at the
end of the session ?
16 bytes per data file. 4 MB would mean about 260,000 MD5s.
A quick glimpse with
    -find / -exec get_any_xattr --
shows that hardlink siblings share the isofs.cx index to MD5.
So if your ISO contains about 260000 different deduplicated
file contents, then the 4 MB would be explainable.
