Having groff natively detect this would indeed be useful.
But some of it turns out to be achievable in user space: see the
clever solution for detecting lines overrunning the right margin that
Peter, Ralph, and Tadziu developed in the thread starting at
http://lists.gnu.org/r/groff/2015-12/msg00049.html.
