Many old PRs contain users that are no longer known by the system. With a new PR you can put a non-existent user on the Cc: list. In both cases it may happen when I submit the PR after editing, that the web server (apache) complains about a malformed header:[Mon Dec 17 13:12:11 2001] [error] [client 193.88.72.107] malformed header from script. This is probably caused by an error message from GNATS itself which Gnatsweb isn't prepared for, and which it therefore doesn't catch. Error handling is an area which has received quite a lot of work in Gnatsweb 4, but a lot remains. I am not in a position to do a full analysis of this now, but you may take a look at your GNATS server setup, because this does indicate that there is a problem in there somewhere.
