They are never removed. The entry is marked  "closed" after the user
disconnects, or to be precise when an accounting record arrives such that
it has Acct-Status-Type = Stop and its NAS-Port-Id, User-Name and 
Acct-Session-Id match those stored in the radutmp entry.

Then either the stop record does not arrive, or it arrives but some of the
above conditions is not met.

The maximum level for debugging the radius protocol is "radius.c"=100.
