This patch is a start at correcting the violation of POSIX reserved
namespace with `_t' suffixes, and the confusion over which prefixes
libltdl uses -- reduced to lt_ and LT_ (except for
LTDL_SET_PRELOADED_SYMBOLS).

At the end of ltdl.h, I define all the old symbols in terms of the new
ones for backwards comptibility.  Actually, I would rather not have
these here at all... but I can't think of a clean way to deprecate the
old symbols.

Okay to commit?
