In the meantime, I've improved it somewhat. The numbers are now accommodated in the vertical spacing. There is some issue of numbers not getting bumped up at times and nestling in too close, but collisions are avoided. (The last two measures of the example show this nestling, and how forcing the accidental bumps the number out of the way.)
You can adjust the height by overriding 'outside-staff-padding. Also, you can have independent simultaneous counters on different staves of a system.
It would be great if anyone interested could try this out and see if there are any issues with it, ways which I could improve it.
