All links of the type
*note (lilypond)...
or
*note (lilypond-internals)...
must be changed to
*note (lilypond/lilypond)...
and
*note (lilypond/lilypond-internals)...
otherwise the stand-alone info reader fails to follow the links.
Tested with texinfo 4.6.93.
