Ok. Have you done a clean web build?
You may take one with missing note-heads, say lily-000000001.eps, and do
lilypond -beps --png lily-000000001.ly
to see if that gives more information.

Strictly not, but if dvips prints feta-related warnings, you won't see
note heads even if the eps is fine.
